4 抗氧化作用
OFR是外层电子轨道上存在非配对电子的离子、分子或基团,其产生来源有:中性粒细胞、线粒体。生理条件下,大约有97%的O 2 在线粒体内被充分还原生成H 2 O,3%左右的O 2 还原不完全生成OFR。同时,机体存在清除OFR的酶系统和小分子还原剂。这样使得OFR的生成和清除处于一种动态平衡。若这种动态平衡遭到破坏时,机体便产生氧化应激,即机体OFR的清除能力下降或(和)OFR的生成增多。大量研究证实,氧化应激参与了脑缺血损伤的发生发展。脑缺血后,能量缺乏,导致钙超载,兴奋性氨基酸释放,激活N-甲基天冬氨酸(NMDA)受体,又进一步加剧钙离子内流,激活钙依赖性黄嘌呤氧化酶,产生大量OFR,OFR对机体的主要损害是导致膜功能障碍,从而造成脑功能与结构的损伤 [16]。一些研究表明,雌激素能防止过氧化 物的堆积,清除OFR,降低神经元对氧化作用的易感性,从而发挥其抗氧化功能。Deng等 [17] 用实验证明雌激素具有强烈的清除OFR的功能。使用雌激素治疗大脑中动脉阻塞模型大鼠,能降低OFR的生成,减轻脑缺血再灌注损伤 [18] 。
5 下调N-甲基天冬氨酸(NMDA)受体的表达,阻断钙离子通道
引起脑缺血再灌损伤的因素很多,但一般认为细胞内钙超载是导致缺血后神经细胞死亡的最终通路。脑缺血后,氧和血糖供应不足,能量生成减少,导致神经元和神经胶质细胞去极化。突触前膜电压依赖性Ca 2+ 离子通道开放,谷氨酸(Glu)释放到突触间隙,促使突触间隙兴奋性神经递质Glu的浓度增高及NMDA受体的过度表达,Glu持续刺激NMDA受体,会导致突触后神经元过度兴奋,继而产生一系列的兴奋性神经毒性。同时大量Ca 2+ 内流导致钙超载,启动神经细胞损伤的一系列病理反应,激活Ca 2+ 依赖性蛋白酶,降解细胞骨架,诱导细胞坏死和凋亡。可见,钙超载和Glu的神经兴奋性毒性是脑缺血损伤的重要环节,而雌激素能对此产生拮抗作用。Weaver等 [19] 研究证明,17-雌二醇能抑制NMDA受体的过度表达,阻滞钙离子内流,减轻脑缺血时谷氨酸和NMDA受体介导的兴奋性神经毒性。Mermelstein [20] 研究发现,雌激素可直接阻断血管和神经的L型电压依赖性钙通道。
6 调节一氧化氮(NO)的生成
NO具有广泛的生物学效应,由L-精氨酸在一氧化氮合酶(NOS)的作用下合成。NOS可分为结构型(cNOS)和诱导型(iNOS),cNOS包括内皮源性(eNOS)和神经源性(nNOS)。NO是一种内皮源性血管舒张因子,在脑内具有舒张血管,抑制血小板聚集的作用,同时,也可以作为神经递质传递信息,参与脑细胞的保护和损伤及脑循环的调节。近来研究提示NO在脑缺血中的作用是双重性的。既能舒张脑血管,抑制血小板聚集,下调NMDA受体的表达,减轻缺血性损害,又能形成自由基产生神经毒性,其原因可能与NO的不同来源及细胞本身氧化还原状态有关。Rubanyi等 [21] 证实雌激素受体(ER)含量与内皮源性NO的水平呈正相关关系。Arnal等 [22] 发现雌激素能抑制氧自由基产物而增加内皮源性NO的生物活性,从而发挥其神经细胞保护作用。
7 调节细胞因子(cytokines,CKs)的生物活性
CKs是指免疫活化细胞和相关细胞产生和分泌的能影响其它细胞或分泌细胞的自身生长、分化和增殖的一类水溶性多肽糖蛋白,它在体内具有广泛的生物活性,能传递细胞间信息,调节细胞功能。大量动物实验证实,脑缺血后,缺氧刺激炎症因子的基因表达,NF-κB、PAF、TNFα、IL-1β、ECAM等大量表达,中性粒细胞与粘附分子相互作用后进入脑实质,脑部炎症反应十分明显,细胞因子在脑缺血再灌注损伤中发挥了十分重要的作用。细胞因子IL-1、IL-8、TNF、PAF等是细胞表面粘附分子的强效诱导剂。若能阻断其释放,则能降低脑缺血再灌注损伤的粘附分子表达,Speyer CL等 [23] 发现雌激素能抑制巨噬细胞的渗出和IL-1的释放。Santizo等 [24] 经实验观察到,雌激素能使血清中I-CAM-1和VCAM-1的表达下调,从而减轻脑循环中白细胞的粘附,降低脑循环栓塞的危险性。脑缺血损伤是一个极其复杂的病理过程,其中涉及到多种机制。治疗时应从多方面进行考虑。近年来的研究表明,雌激素能通过多种机制对脑缺血损伤发挥保护作用,立即显露出良好的应用前景。但由于动物或人类长期使用雌激素有增加乳腺癌和子宫内膜癌的风险,而抑制了其应用。若能利用另一种药对抗其副作用,必将大大推广雌激素的应用。此外,临床上运用雌激素替代疗法,缺乏统一的疗效评价,容易受年龄、饮酒及情绪抑制等诸多干扰因素的影响。因此,设计一个大样本、多中心、随机、双盲、安慰剂对照的干预试验十分必要。雌激素水平与脑缺血损伤发生、发展的内在机制以及雌激素的用量和副作用等问题有待进一步探索。
